The FUHSA Senate, in line with JAMB policy, has set the following minimum UTME scores for the 2025/2026 academic session:

MBBS (Medicine) & BDS (Dentistry) is 220 and above
Nursing Sciences, Optometry, Physiotherapy, Radiography, Medical Laboratory Science (BMLS), Audiology is 200 and above
Nutrition & Dietetics; IT & Health Informatics; Environmental Health Sciences; Epidemiology & Biostatistics; Speech-Language Therapy; Occupational Therapy; Health Information Management; Public Health; Pharmacology has 160 and above
Human Anatomy, Human Physiology, Biochemistry, Biotechnology, and Microbiology have 150 or above.

Eligibility Criteria for the Federal University of Health Sciences Azare Post UTME Form 2025/2026

You can apply for the FUHSA Post UTME Form 2025 if you:

Sat for the 2025 UTME, met the cut-off for your chosen course, and selected FUHSA as your first choice.
Have at least five (5) relevant O’Level credits, including English Language, Mathematics, Physics, Biology, and Chemistry, in not more than two sittings.
Wrote the correct UTME subject combination for your course.
(For Direct Entry) Possess the appropriate qualifications and O’Level results as required by the program.

Note: Your O’Level results must be uploaded on the JAMB CAPS portal before admission can be processed.

How to Apply for the FUHSA Post UTME Form 2025/2026

Follow the steps below to apply:

Visit www.fuhsa.edu.ng for detailed instructions.
Go to the FUHSA admissions portal at https://fuhsa.admissions.cloud.
Create an account on the portal.
Pay the ₦2,000 (non-refundable) screening fee online.
Complete the online application form and print your acknowledgement slip, which contains your physical screening date.

Required Documents for Physical Screening

On your screening day, go with both the originals and photocopies of the following:

JAMB Result Slip (showing your photo).
O’Level Result(s) (WAEC, NECO, or equivalent).
Acknowledgement Slip from FUHSA Post-UTME registration.
Birth Certificate or Declaration of Age.
Local Government Area (LGA) Indigene Certificate.
Valid Means of Identification (e.g., National ID card, NIN slip, or international passport).
For Direct Entry candidates: Bring your DE registration slip, plus relevant certificates (e.g., ND, NCE, HND, A-level results).
